It depends what you value more.
A fitness specialist makes your players Stanima drop slower while a career extension coach makes all the other skills drop slower. So the career extension trainer is good if you have an aging squad while the fitness specialist is good no matter what IMO

Sammy's right, depends on the age of your squad. It's also not super important to get one with a specialty like that, it'll cost you a bit more than one without, and I don't think either of those are too special. But it's your choice.

Yeah from my experience I went from having a fitness expert to one with no speciallty and didn't notice much of a difference. I do have a couple more drops in Stanima but overall its not much difference.
